Pour la prévention de l'angine de poitrine due à une maladie coronarienne.
Iso-Bid Pharmacologie
Dinitrate d'isosorbide est modéré à longue durée d'action nitrates organiques utilisés par voie orale pour le soulagement et le traitement prophylactique de l'angine de poitrine. Il détend le muscle lisse vasculaire et la dilatation consécutive des artères et des veines périphériques, surtout la dernière. Dilatation des veines périphériques favorise l'accumulation de sang et diminue le retour veineux vers le cœur, réduisant ainsi ventriculaire gauche diastolique pression et la pression capillaire pulmonaire (précharge). Relâchement des artérioles réduit la résistance vasculaire systémique, la pression artérielle systolique et la pression artérielle moyenne.
Iso-Bid Absorption
L'absorption de dinitrate d'isosorbide après administration orale est presque complète, mais sa biodisponibilité est très variable (10% à 90%), avec un effet de premier passage hépatique dans le foie. La biodisponibilité moyenne de dinitrate d'isosorbide est d'environ 25%.
Iso-Bid Toxicite
Les symptômes de surdose comprennent la réduction du débit cardiaque et une hypotension.

PATIENT INFORMATION
Patients should be told that the anti-anginal efficacy of isosorbide dinitrate is strongly related
to its dosing regimen, so the prescribed schedule of dosing should be followed carefully. In
particular, daily headaches sometimes accompany treatment with isosorbide dinitrate. In patients
who get these headaches, the headaches are a marker of the activity of the drug. Patients should
resist the temptation to avoid headaches by altering the schedule of their treatment with isosorbide
dinitrate, since loss of headache may be associated with simultaneous loss of anti-anginal efficacy.
Aspirin and/or acetaminophen, on the other hand, often successfully relieve isosorbide dinitrate-
induced headaches with no deleterious effect an isosorbide dinitrate's anti-anginal efficacy.
Treatment with isosorbide dinitrate may be associated with lightheadedness on standing, especially
just after rising from a recumbent or seated position. This effect may be more frequent in patients
who have also consumed alcohol
